Honoring Choices is a non-profit organization focused on the rights of every adult to make choices and communicate these choices in a personal care plan. The personal plan reflects an individual’s likes and dislikes for the kind of care one wants and needs at every phase of health. Making a plan helps everyone – the adults, families and care partners, and importantly, the doctors and care teams, in delivering care that matches the person’s care preferences today and every day over their lifetime. This evening’s event will include:
- Massachusetts law and the rights of every adult to plan for their future
- How to begin creating your own personal plan with the Honoring Choices Getting Started Tool Kit and Adult & Supportive Person Tool Kit
- Sharing free downloadable documents and tools: Health Care Proxy (14 languages) and Personal Directive (Living Will) (5 Languages); “I Have a Health Care Proxy” Wallet Card (4 languages)
- Reviewing of five Massachusetts care planning documents
- An overview of the basics of Guardianship, Conservatorship and alternatives
- We will talk about issues related to aging and chronic and serious illness and how MA law presently addresses choices that guardians may want to exercise for the person who is ill.

Ellen M. DiPaola, JD, is the President and CEO of Honoring Choices Massachusetts. Ms. DiPaola had a law practice with a focus on Guardianship and Conservatorship. Ms. DiPaola oversees all aspects of the organization and convenes the New England Healthy Living Collective and the Consumer Health Equity Group. She chairs the Guardian-Serious Illness Advisory Group, co-sponsored by the Massachusetts Health and Hospital Association.
